Notice is hereby given that Southern California Gas Company (SoCalGas) has made the customer- specific information described below available to one or more of its affiliates. SoCalGas does not release information to any third party, including its affiliates, unless it first obtains prior written consent from that customer to do so. This notice will have a general description of the type of data to be released (for example data regarding energy usage), but the notice will not disclose the customer's name, nor will it disclose the actual data. This information will remain posted for 30 days.

Please note: provision of customer-specific information associated with acceptance of an electric direct access service request or a gas direct access service request for core aggregation are covered by Rules 25 and 32, respectively, and do not require this form to be submitted.
